Goserelin acetate
Indications/Uses
Zoladex/Zoladex LA Prostate cancer for hormonal manipulation. Zoladex Advanced breast cancer in pre- & peri-menopausal women suitable for hormonal manipulation; alternative to chemotherapy in the standard care for pre-/peri-menopausal women w/ oestrogen receptor (ER)-positive early breast cancer. Endometriosis, endometrial thinning, uterine fibroids, assisted reproduction.
Dosage/Direction for Use
Zoladex Adult 1 depot SC inj into the anterior abdominal wall every 28 days. Assisted reproduction Administered as defined by serum oestradiol levels. Endometriosis Max: 6 mth. Endometrial thinning 2 depots administered 4 wk apart, w/ surgery timed between 0 & 2 wk after 2nd depot. Uterine fibroids Anaemic women 1 depot w/ supplementary Fe for up to 3 mth pre-op. Elderly, renal & hepatic impairment No dose adjustment necessary. Zoladex LA Adult male, elderly 1 depot SC inj into the anterior abdominal wall every 12 wk. Renal or hepatic impairment No dose adjustment necessary.
Contraindications
Hypersensitivity to goserelin acetate or to other LHRH analogues. Pregnancy & lactation. Childn. Zoladex LA Female.
Special Precautions
Zoladex/Zoladex LA May prolong QT interval. Not indicated for use in childn. Zoladex Reduced bone mineral density. Male Patients at risk of developing ureteric obstruction or spinal cord compression. Reduced glucose tolerance, increased risk of developing MI, sudden cardiac death & stroke. Female Metabolic bone disease, uterine cervical resistance, risk of developing ovarian hyperstimulation syndrome (OHSS). Polycystic ovarian syndrome. Zoladex LA Men at risk of ureteric obstruction or spinal cord compression. Reduced glucose tolerance; increased risk of developing MI, sudden cardiac death & stroke. Not indicated for use in females.
Adverse Reactions
Decreased libido, hot flushes, hyperhidrosis, paraesthesia, abnormal BP, rash, inj site reactions, decreased bone density, increased wt. Male Erectile dysfunction, impaired glucose tolerance, mood swings, spinal cord compression, cardiac failure, MI, bone pain, gynaecomastia. Female Acne, vulvovaginal dryness, breast enlargement, altered mood & depression, headache, alopecia, arthralgia, tumour flare & pain.
Drug Interactions
Potential additive effect on QT interval w/ medicinal products that prolong the QT interval or medicinal products able to induce Torsade de Pointes eg, class IA (eg, quinidine, disopyramide) or class III (eg, amiodarone, sotalol, dofetilide, ibutilide) antiarrhythmic medicinal products, methadone, moxifloxacin, antipsychotics.
